• Completed Signed Registration Packet
• Certified Copy of the Birth Certificate (ARS 15-828)
• Proof of immunization (ARS 15-872)
• If applicable:
• Power of Attorney if the student is not living with the parent or legal guardian
• Custody Papers
• Restraining Order
