Health News from the School Office

Emergency Contact Information                       

Notify the school office when you change: Phone numbers (home, work or mobile); Pager numbers; or home address. Also, notify the school office if there are changes of names and/or phone numbers for the persons you authorized to be contacted if you cannot be reached. It is important that the school be able to contact you if your child is sick or injured.


Medications

Students are not allowed to self-medicate while on campus.  Children who require medications during the day must have written parent/guardian permission on a school medication form. Prescription medication must be provided to the office in the current, original container with the prescription label for that child. Over-the-counter medication must be provided in the original container and a school medication form completed by the parent/guardian. Medication needs to be kept in the school office.

Immunizations

 Shots and Health Records Needed
State law requires students to have immunizations to attend school. Please bring immunization records at the time of registration for review. Contact the school immediately if there are any questions. Locations for free shots are available form the office.
